To earn continuing education credits related to Special Needs Plans (SNPs), go to OptumHealth Education (OHE) where you will find a 7 module series titled “The SNP Learning Path”. This series is relevant to physicians, physician assistants, nurses, nurse practitioners, case managers, social workers and other health care professionals who have an interest in SNPs.

If your organization uses ICUE to access the HRA, the module “Accessing the HRA and ICP in ICUE” can be accessed in Provider Development Institute.

Note: This module is operational and therefore does not provide continuing education credits and is not part of the series in OHE.

These courses focus on the care management of vulnerable populations and are founded on the CMS Special Needs Plan (SNP) Model of Care (MOC).  The MOC is the basic framework that helps to ensure that each SNP will meet the needs of its members. These courses explain the various components of the MOC and how they help to identify and meet the unique needs of each member. The goal of the MOC is to promote quality care management practices and care coordination processes.

Note: The SNP Learning Path does NOT replace the SNP Model of Care and Final Rule trainings that are required annually by a health plan.
